The full form of ICT is Insulin shock therapy or Insulin Coma therapy. This is a psychiatric treatment in which patients are continuously injected with a heavy dosage of insulin. This in turn results in comas for several weeks. In the first decades of the 20th century, ICT therapy was used.

Insulin shock therapy or Insulin Coma therapy is generally known as ICT in short. This ICT was first presented in the year 1927. Mainly this is psychiatric management in which patients are endlessly injected with a heavy dosage of insulin. Additionally, it includes Electroconvulsive treatment as well as provides deep sleep therapy too.

Furthermore, this ICT was first introduced in the year 1927 by Austrian-American Psychiatrist Manfred Sakel. However, this method was majorly used in the 1940s to 1950s for Schizophrenia. And in the 1960s for a replacement of Neuroleptic drugs too. As mentioned few therapies which is included are convulsive therapies, which include Metrazol, carbimazole therapy. Additionally, it includes Electroconvulsive therapy as well as provides deep sleep therapy too. Especially Insulin coma therapies and convulsive are the shock therapies which is used. In general, this therapy needs a special unit as well as trained people too. Therefore, patients diagnosed with schizophrenia will be selected on basis of their physical strength to withstand the treatment. But still, there are no guidelines established and generally, injections are used for 6 days a week for continuous 2 months.

Why was Sakel include insulin for this therapy?

“My supposition was that some noxious agent weakened the resilience and the metabolism of the nerve cells. A reduction in the energy spending of the cell, that is in invoking a minor or greater hibernation in it, by blocking the cell off with insulin will force it to conserve functional energy and store it to be available for the reinforcement of the cell.”

In the year 1927, Sakel a medical doctor in Vienna discovered the treatment. He was working in a psychiatric clinic in Berlin. There was a patient who experienced improved mental clarity after having slipped into an accidental coma. After that Sakel, found the treatment might work for mentally ill patients.

In the treatment of 182 cases of Schizophrenia, Insulin Coma Therapy (ICT) was found to be very effective. Some of the other therapies include deep sleep therapy, psychosurgery cardiazol or metrazol therapy, electroconvulsive therapy, etc. Shock therapies are basically a combination of Insulin coma therapy and convulsive therapies.
